Levothyroxine is a synthetic T4 thyroid hormone with a very long plasma half-life of about 7 days in people with normal thyroid function, so its blood level barely moves from one daily dose to the next. That slow clearance is why it is taken once daily and why a dose change takes about 6 weeks to reach a new steady level. T4 circulates as a large, tightly protein-bound reservoir that the body converts to T3 gradually, which is what gives it such a long and stable half-life.
